An Unusual Case of Colitis in a Bone Marrow Transplant Patient



Question: A 65-year-old man with multiple myeloma status after bone marrow transplantation 5 years before presentation, controlled with lenalidomide and daratumumab, presented to the gastrointestinal (GI) clinic with worsening diarrhea, epigastric abdominal pain, nausea with vomiting, and 45 pounds weight loss over the past 6 months. One month before the clinic visit he was hospitalized with fever, abdominal pain, nausea with vomiting with acute kidney injury, hyponatremia on laboratory tests, and contrast computed tomographic abdomen/pelvis findings as shown (Figure A, B).
